I understand that SlimeTime & More activities involve tactile, sensory materials including slime bases, glitter, scents, foam beads, and charms. I confirm my child is able to participate safely and does not have known allergies or sensitivities to these materials (beyond what is noted above). I give permission for my child to participate fully in all SlimeTime & More activities.
I give permission for SlimeTime & More to photograph and/or video record my child during the event. These images may be used on social media, the website, and marketing materials for promotional purposes only. No child's full name or personal information will be shared publicly.
I, the undersigned parent or legal guardian, hereby release SlimeTime & More, its owners, employees, and volunteers from any and all liability, claims, demands, or causes of action that may arise from my child's participation in SlimeTime & More activities, including but not limited to any skin reactions, allergic responses, or minor injuries that may occur during normal participation. I acknowledge that I have disclosed all known allergies and medical conditions relevant to this activity. This waiver is binding upon me and my heirs, assigns, and legal representatives.
